Anesthetic Management for Whole Lung Lavage in a Patient with Pulmonary Alveolar Proteinosis




Abstract
Pulmonary alveolar proteinosis is a rare disorder in which an amorphous, insoluble, proteinaceous material, rich in phospholipid, deposit in alveoli and bronchioles. The primary disturbance produced by this disorder is impairment in oxygen
transfer
to
the blood perfusing the involved alveoli. This disease was first described in 1958 by Rosen et al. The cause of this disease is still unknown.
Whole lung lavage is the only successful treatment for this disease. The first case of pulmonary alveolary proteinosis treated by whole lung lavage in Korea was reported in 1989.
We report our experience of the anesthetic management for whole lung lavage in 47 year old patient who was diagnosed as pulmonary alveolar proteinosis. At first, whole lung lavage of left lung was carried out succesfully, followed by right lug
lavage 1
week later. After treatment, he showed improvement in symptom and the lesion on the chest CT decreased.
